Required forms for raffles, bazaars
What You Need:

        • Application Form (3 copies)
        • Annual Report Form (3 copies)
        • One Green Form
        • One Tax Form
        • ** First-time applicants must submit a statement of organization or bylaws.

Forms must be approved and signed by the Director of Police Services (police chief)
All completed forms must be returned to the Town Clerk’s Office for signature and issuance of permit.

Cost is $50.

Expiration: The permit expires one year from date of issuance.

Applications:

        • One copy remains in Town Clerk’s Office.
        • One copy is for the applicant.
        • The clerk's office mails one copy to the state.

Annual Reports:

        • One copy remains in Town Clerk’s Office.
        • One copy to applicant.
        • The clerk's office mails one copy to the state.

Green Form: 
Green Forms must be picked up at the Clerk's Office. The Clerk's Office certifies and send a copy to the state.

Tax Forms:
Tax Forms can be obtained from the Clerk's Office.

Then print them, fill them out as completely as you can and bring them to the Town Clerk's Office, second floor, Town Hall, 730 Mass. Ave. These forms are not official until they are signed and certified in the clerk's office. Other required forms are available at the clerk's office.
